HCP is a state-mandated, state-funded, locally managed program to served the needs of children with birth defects and disabilities. Although these problems arise across the full spectrum of society, this program is targeted at those children in the area who have less access to care and therefore less access to specialty services. The goal of the program is to provide help so that further complications may be prevented for children with disabilities. To be seen a child must first apply for Medicaid or CHP+.
